Contact Your Doctor – If you are experiencing pain or other symptoms after an accident or injury; if you have a question regarding a drug injury, side effect, or interaction; if you are experiencing symptoms of food poisoning; if you have been harmed by a defective medical device, or consumer product; or if you have any other questions regarding your heath please consult with you doctor or healthcare professional, or if necessary visit an emergency room immediately.

Follow Your Doctor’s Advice – Your health is your doctor’s primary concern. By following your doctor’s advice, you not only show your commitment to your well being, but you also make it possible for your doctor to develop a record of your condition. Do not miss doctor’s appointments. Do not fail to inform your doctor about your symptoms. Always be honest with your doctor; if you cannot remember something, tell your doctor.

Regarding Your Information:

Keep track of your records – We will ask you for contact information for all of your doctors. We will also ask you for information regarding any hospital stays, drugs taken, and medical bills. This information is very important to help us prepare your case. We will ask you to sign written authorizations to have all those who treated you furnish us with copies of your medical records. It is important that you provide us with these authorizations at the beginning of your case.

Providing evidence – Depending on the nature of your injury, we may ask you for the following information or evidence in your possession:

  • Contact information for witnesses to your accident.
  • Photographs or video related to your accident or injury. Use your cell phone camera if necessary.
  • Contact information for doctors, hospitals, and/or pharmacies.
  • Medical bills and records.
  • Medical paraphernalia (e.g., prescription bottles, etc.)
  • Receipts for expenses related to injury.
  • Any physical evidence (e.g., defective product, clothing or other items worn at time of accident, etc.)

Keep detailed records – It is important to keep detailed records about the following:

  • Missed work time and compensation.
  • Expenses related to your injuries.
  • The extent of your injury, pain and suffering, and any physical limitations.
